Committing our children to the Lord

When you present your child to God in the form of a child dedication, you are publicly recognizing that your child belongs to Him and that you will raise the child in a home committed to experiencing and loving Christ. You are also committed to following biblical principles and teaching the word of God in your home.

Deuteronomy 6:5-7 says this, ”And you must love the Lord your God with all your heart, all your soul, and all your strength. And you must commit yourselves wholeheartedly to these commands that I am giving you today. Repeat them again and again to your children. Talk about them when you are at home and when you are on the road, when you are going to bed and when you are getting up.”

Proverbs 22:6 says this “ Train up a child in the way he should go, and when he is old he will not depart form it.

As Christian parents, you are committing to teaching your child the truth found in God’s word and practicing the disciplines of faith. You are making a public declaration that you believe in biblical principles and are practicing them on a daily basis.

Before filling out this application please read the Foursquare/New Life Church requirements to participate in a child dedication service.

  • You are attending church on a regular basis.
  • You have acknowledged Christ as your Lord and Savior.
  • You are following biblical principles in your daily life.
  • You believe the word of God is true.
  • You must be the parents or legal guardian of the child.

Child Dedication




  • Due to the spiritual nature of the questions asked during the Baby Dedication Ceremony, it is helpful for the Pastor to understand the marital and living situation of both parents.

    Which of the following best describes the parents’ marital and living situation?

  • *We ask parents who are living together and are not married to take steps to acknowledge and surrender to God’s plan for their individual lives before participating in a Child Dedication service.







  • We do our best to accommodate special requests, such as dates and pastor(s) requested. If you do not have a preference, you will be scheduled at the next available dedication with a qualified pastor. All of our pastors are very excited to take part in such a meaningful ceremony.



  • There are a few Foursquare guidelines we ask the parents to support before we will commit to dedicating your child(ren) in our gatherings. Please make sure all of the following are true of your situation before submitting this form:

    • We ask parents to have a personal relationship with Jesus Christ.
    • You must be the parents or legal guardians of the child.
    • We require parents who are living together and are not married to acknowledge and surrender to God’s plan for their individual lives before participating in a public declaration to follow biblical principles.
